R-1 Sponsorship Opportunities

This form provides our standard benefits for your generous donation.  If you would like to sponsor a specific section of the Corona Lemon Festival or customize your sponsorship contact Debbie Morgan at debmorgan.lemonfest@gmail.com.

 

R-2 Vendor Application

This form is to be completed by all vendors including food vendors.

R-3 Vendor Terms (2 pages)

This form must be signed and attached to all vendor applications.  It should be read carefully as it contains information and instructions for what we expect of our vendors

R-4 Opportunity Drawing Donation Form

This form is to be submitted for any items donated to establish the value for tax purposes.  Please attach your business card.  We are asking all vendors with product to make a donation to our opportunity drawing if possible.

 

R-5 In-Kind Donation

This form is to be submitted to establish the value for tax purposes of any items donated to be used for the production of the festival.

R-6 Food Vendor Health Department (3 pages)

These forms must be filled out by all food vendors cooking on site.

Food vendors bringing pre-cooked food must provide proof of Health Department Certified Kitchen.

R-7 CDTFA 410-D Special Events Certification   Request this Form from Mary

This form is for tax exempt distributors whose corporate entity pays the sales tax such as Avon and Origami Owl.

 

Sample Certificate of Insurance

Follow the instructions located in the Vendor Terms exactly.

You may purchase event specific insurance through Corona Art Association if you don’t have your own adequate coverage.
